Quality of Care

Due to considerable overlap, this workgroup was constituted in 2009 as an amalgamation of two earlier workgroups, the Integration Workgroup and the Individual and Family Care Workgroup. The goal for this combined workgroup is to take the lead in coordination and integration of HPC services across the Central LHIN. The intended outcomes of this workgroup, now called “Quality of Care workgroup” include:

  • Access to Hospice Palliative Care services across the Central LHIN is facilitated.
  • Consistent outcomes of Hospice Palliative Care services are routinely measured and communicated.

In order to ensure that access is facilitated, the workgroup proposes to:

  • Identify local services, including service elements and characteristics,
  • Communicate availability of HPC services,
  • Ensure ease of entry to the system, and
  • Ensure tracking of individuals using the system.

In order to ensure that outcomes are measured and communicated, the workgroup proposes to:

  • Identify initial outcomes of interest to Hospice Palliative Care services across the Central LHIN, together with the Research and Evaluation Workgroup,
  • Compare these initial outcomes of interest to ensure consistency of outcomes across the Central LHIN,
  • Communicate these outcomes, together with the Communication and Advocacy Workgroup, and
  • Promote a process for ongoing development and review of outcomes of interest to ensure consistent outcomes across the Central LHIN.
