LEAP Core 2019
Learning Essential Approaches to Palliative and End-of-Life Care (LEAP) Core is Pallium Canada's 2 -day program offering competency based palliative care education for health care professionals. This program, led by LEAP certified facilitators, is based on Canadian realities and current best practice in palliative and end of life care.
  • $175.00 for Physicians and $115.00 for all other professionals
  • No pre- requisites: this course is only for professionals working in the Central LHIN
  • Attendance is mandatory for physicians to receive certification with the CFPC for up to 28 Mainpro + credits
  • Next session available in the Central LHIN is at Margaret Bahen Hospice, 653 Queen St, Newmarket, ON L3Y 2J1
    June 8 and June 9, 2019 from 0800 hours to 1700 hours
  • Breakfast and lunch provided within the registration cost

Fundamentals 2019
Fundamentals of Hospice Palliative Care is an excellent foundational palliative care program for nurses to develop assessment, problem- solving and critical thinking skills.
  • Cost is $65.00
  • Target audience includes all nurses: RNs, RPNs and NPs
  • No pre-requisites required
  • 3 face to face half day classes and one 2 hour coaching session
  • Next session starts May 1, May 15, May 29 from 0900-1230 hrs

CAPCE 2019
Comprehensive Advanced Palliative Care Education (CAPCE) prepares nurses who are
interested in becoming hospice palliative care resource nurses within their health care team.
  • Cost is $325.00
  • Target audience includes all nurses: RNs, RPNs and NPs
  • Pre-requisites: Must have 1 year of experience in palliative care and
    additional education in palliative care. Preference for Fundamentals of Hospice Palliative Care
    although other courses will be considered on an individual basis, including:
    LEAP and the CNA certification in Hospice Palliative Care
  • Attendance is mandatory to receive certification
  • Next session: 0800 to 1200 hours, January 22, February 26 and April 2, 2019
  • 9 two hour coaching sessions approximately 2 weeks apart will be organized
  • 8 peer to peer sessions organized individually after the first class

Building Capacity
  • HPC Teams facilitates a one day session for selected LTCHs who apply
    through an annual Expression of Interest. The program focuses on providing quality
    end of life care to LTCH residents through interprofessional teamwork.

LEAP LTC
Learning Essential Approaches to Palliative and End-of-Life Care (LEAP) Long Term Care
is a palliative care course for all healthcare providers working in long term care homes.
This program reviews the essential competencies for a palliative approach to care
while promoting teamwork within the LTCH.
  • Fall 2018 HPC Teams is teaching one LTCH LEAP sessions:
